Whanau Ora – Diabetes Nursing

Our Aim Working with our health promotion team we encourage Maori patients with diabetes to self manage their disease.

We promote awareness of diabetes in the far north by providing health promotion and education relative to diabetes within the community, and provide access to Diabetes Annual Review services via outreach.

Objectives

  • Providing diabetes care;
  • Coordinating and providing clinical resource and support within Kaupapa Maori philosophy to turoro;
  • Leading delivery of quality awareness raising activities for at risk groups;
  • Ensuring Maori with diabetes participate in a progress of annual reviews (the Get Checked programme);
  • Providing accessible and acceptable community based specialty diabetes nursing service targeted at Maori and focused on reducing inequalities;
  • Providing education to primary health providers to ensure clinical practice in diabetes is current and evidence based;
  • To liase closely with Northland Health Diabetes Centre, Whangarei Hospital and the Diabetes Nurse Specialist based at Kaitaia Hospital.

Eligible Clients Any patients diagnosed with Diabetes (Type 1 or Type 2).
