Whanau Ora – KMACC Nursing
Our Aim This highly specialised service is delivered by advanced nurses aimed at high-risk patients with chronic health conditions. Improving access to services means patients receive intensive support and enable them to better manage their total well-being. We work in partnership with individual whanau, hapu and iwi to deliver the most culturally appropriate health services to the rohe of Te Hiku O Te Ika. We aim to enhance the vision of wellness and empower whanau towards better self-management of health.
Objectives
- Client assessment and nursing treatment;
- Development of individual self management care plans, involving these identified by the individual as significant to supporting their ability to self manage;
- Integration and collaboration with primary and secondary health care services, other health organisations, disability or social services;
- Health advice and follow up care;
- Health promotion and education;
- Ongoing monitoring of chronic condition or other relevant issues;
- Follow up on hospital discharges to those clients referred to our services;
- Follow up care from GP services to clients enrolled with Te Hauora O Te Hiku O Te Ika.
Eligible Clients Access to this service is managed on clients chronic care conditions and priority of need and capacity to benefit. This service is aiming to reach those clients and whanau who require information and support to effectively manage their chronic condition.
Potential clients will be enrolled, assessed and prioritised as to the extent of the service required. If clients currently using the service are seen to be managing their chronic conditions effectively they will receive minimal follow-ups or get discharged to the GP services.
All clients enrolled with Te Hauora O Te Hiku O Te Ika are eligible.
