Whanau Ora – Mobile Nursing

Our Aim We aim to enhance the vision of wellness and to empower whanau towards better health. We provide ongoing nursing education, assessment and treatment to maintain optimum health. To enhance self-reliance we promote awareness, disease prevention and management across the lifespan. The team shall promote awareness and commitment to honouring the Treaty of Waitangi at all times.

Objectives

  • Identification of clients at risk of illness or injury;
  • Client assessment and nursing treatment;
  • Development of individual care plans with whanau involvement;
  • Collaboration with members of the primary and secondary health care teams or other relevant health, disability or social agencies;
  • Health advice and follow up care;
  • Health promotion and education;
  • Ongoing monitoring of illness or other relevant issues;
  • Follow up on hospital discharges and GP services to clients who have chosen our services.

Eligible Clients All whanau, iwi and hapu of Te Hiku O Te Ika.

How do I access this service?

  • Self referral;
  • Whanau, hapu, iwi members;
  • Hospital discharges;
  • Other allied Maori health providers;
  • Other allied providers - e.g. community groups and primary/secondary care.
  • Referral from Te Whare Hauora clinic by GP's or nurses.
