Respiratory Nursing

Our Aim To work in partnership with kaumatua, kuia and whanau to deliver the best possible respiratory health services to the rohe of Te Hiku O Te Ika. To provide ongoing nursing education, assessment and treatment to enhance the vision of wellness, empowerment, self-reliance and self-management for overall optimum health. The team shall promote awareness and commitment to honouring the Treaty of Waitangi.

Objectives

  • Identification of clients at risk of illness;
  • Client assessment and nursing treatments;
  • Development of individual care plans with whanau involvement;
  • Health advice and follow up care;
  • Health promotion and education;
  • Ongoing monitoring of illness or other relevant issues;
  • Follow up on hospital discharges to those clients who have chosen our service;
  • Follow up care from our GP for those clients who have chosen our service;
  • Collaboration with members of the primary and secondary health care teams and other relevant health, disability or social agencies.

Eligible Clients Tamariki -taitamariki 0 - 18 within Te Hiku's service boundary with respiratory conditions.

  • Self referral;
  • Whanau, hapu, iwi members;
  • Hospital discharges;
  • Other allied Maori health providers;
  • Other allied providers - e.g. community groups and primary/secondary care.
